Side-by-side or top-to-bottom field label alignment You have the option of having your field labels directly above the field or to the left of it.

Multiple Column Forms When you choose 'top-to-bottom field label alignment', you also have the option of having multiple columns on your form. Simply drag an item to the right of an existing item and a new column will appear.

Font choices & sizes Choose from a multitude of font & font sizes for your labels, comments & titles.

Free image hosting Because we allow you to upload logos & images from your site, the pages we host become a seamless continuation of your pages. Your clients will not know they have left your site.

Color Customization Modify a multitude of text and background colors, on your form, to match your site.

Page Style Templates We offer 5 different page style templates, allowing you to closely match the format of your own web site pages.

Custom Form Header/Footer This option allows you to create, copy and/or customize portions of HTML code (header/footer) that will surround the main body of your form. This feature will allow a higher customization level than the 5 page style templates offered. Some HTML knowledge is required.

Form Converter The ultimate in customization. Convert an existing form to work with our system. The converted form retains all of it's original display characteristics, while allowing us to process the results, securely, behind the scenes. Appointment Scheduling Define appointment time slots that become unavailable as your clients make selections. Eliminate the need for the manual scheduling of appointments. When your client selects an appointment date & time, they know with confidence that you will be available, because the time slot was defined by you. Form Elements (Input Fields)

Text Box A single-line input field, that can capture a relatively small answer, made up of free form text.

Drop Down Box The selection Box creates a menu from which you are allowed to select one option.

Multiple Selection Drop Down Box Same look as the regular drop down box; however, your client can use the shift & CNTL keys to select more that one option on the list.

Check Box Checkboxes are on/off switches that may be toggled by the user.

Radio Buttons Radio buttons are like checkboxes except that they are mutually exclusive: when one is switched 'on', all others within a grouping are switched 'off'' .

Text Area A multiple-line input field, that can capture a relatively large answer, made up of free form text. This field type also includes a character counter and the ability to set a character limit.

File Selection This input type allows the user to select files, from their computer, so that their contents may be submitted with a form. Your clients may upload & store images and documents with this feature (e.g. A resume in MSWord format). Only one file selection is allowed per form with the basic package. The standard package (& above) will allow up to 5 file selections per form.

Password Field A single-line input field, created exclusively for passwords, which allows the entry of free form text while concealing the true identity of the characters.

Sub-title Lines make it possible to group & describe like line items together.

Comment Lines make it possible to document your form and also ask lengthy questions.

Link Lines make it possible to embed URL links in the body of your form. Perfect for help & agreement web pages.

Spacer Lines make it possible to add blank lines to improve the readability of your form.

Imbedded Images make it possible to add an image to the body of your form, to be used as a product/service illustration or anything you wish.

Product Selection Lines contain items grouped together for the specific purpose of selling products. The elements are made up of a selection box with product descriptions & prices; a selection box with quantity; and a text box that calculates the total line amount. Javascript calculations are performed after each product selections is made, re-calculating all self-totaling items on the page instantly. Multiple Product Selection Lines are are allowed per page. Product Checkbox Lines contain elements grouped together for the specific purpose of selling products. The elements are made up of a checkbox for selecting the product; a fixed text description of the product & price; a selection box with quantity; and a text box that calculates the total line amount. Javascript calculations are performed after each product selection is made, re-calculating all self-totaling items on the page instantly. Multiple Product Checkbox Lines are allowed per page. Product Free Form Lines contain elements grouped together for the specific purpose of selling products. The elements are made up of a text box for entering a product description; a text box for entering a price; a selection box with quantity; and a text box that calculates the total line amount. Javascript calculations are performed after each product selection is made, re-calculating all self-totaling items on the page instantly. Multiple Product Free Form Calc Fields are allowed per page. Example: Food item:

Sales Tax Line is used to calculate sales tax for customers that reside in the state of your choice. Calculations are performed after each product selection is made and based on the percentage of your choice. The percentage is applied to the sub-total and reflected in the grand total instantly. Example: AZ Tax (8.1%): Important: Sales tax will not calculate without a State: drop down box, along with the assigned state selection by the customer.

Ship/Misc Addition Line is used to calculate the shipping & handling of your items. Calculations are performed after each product selection is made and based on the percentage of your choice. The percentage is applied to the sub-total and reflected in the grand total instantly. `Ship/Misc Addition Line` can also be a fixed dollar amount instead of a percentage. Example: Shipping:

Ship/Misc Selection Lines are used to add or subtract shipping & handling, discounts, promotions or any other fixed amount selection from/to the sub-total and reflected in the grand total instantly. Example: Senior Discount:

Sub-Total Line is used to total up your shopping cart line items. Calculations are performed and applied after each product selection is made. Example: Sub Total:

Coupon Code Lines are used to subtract, an amount or percentage, from the sub-total. if the customer knows one of the defined `Coupon Code(s)` and enters it, the system will subtract the corresponding amount or percentage. You can define up to 5 coupon codes along with their associated rate/amount.

Misc % Selection Line is used to add or subtract any miscellaneous percentage to the subtotal. Your client makes a selection from the options provided by you. Then, the corresponding calculated amount will automatically be added or subtracted to/from the total. Example: Value added Tax:

Grand-Total Line is used to total up your shopping cart line items, sales tax, discounts and shipping. Calculations are performed and applied after each product selection is made. Example: Grand Total:

Page Cart converts your self-totaling items to work in an on-page shopping cart.
When Page Cart is selected, the self-totaling function happens within an interactive pop-up box displayed right on top of the page. All self-totaling product fields now have `Add to Cart` buttons and all totals, taxes, coupons & discounts are displayed inside the pop-up shopping cart. Click to to watch it work

Required Entry An option to make certain form elements mandatory, ensures your clients enter all of your required data.

Optional Form Element Edits ensure your clients enter only valid data. Both Javascript & server side edits provide double validation for entered data.
- Numbers Only This will validate that your clients enter only numeric data.
- Valid Date Date validations in the format of mm/dd/yyyy, dd/mm/yyyy or yyyy/mm/dd; depending on your selection.
- Valid Email Validates that your clients enter a valid email format.
- Credit Card Validates Amex, Discover, Master Card, & Visa card formats; in addition to encrypting the number in storage.

Page Min/Max Limits Allows forms to specify an upper and/or lower limit on any field that: a) has a numeric edit b) is a calculated field or c) is one of the following self-totaling components: 1) line qty 2) line amount 3) sub total 4) grand total.

Hidden & Read Only Text Fields Make any text field read only (protected) or hidden. Hidden fields are perfect for passing values from form to form without your client's knowledge. Read only fields are perfect for passing values your clients can see but can't modify.

Prefill Text Fields Prefill any text field with a word or phrase of your choice. There are two methods available: 1. Prefill data by passing it as a parameter in the URL or 2. Add your fixed prefill verbiage as part of the field definition.

Today`s Date Prefill Prefill any text field with today`s date. Using the `read-only` function along with this function ensures that today`s date passes through to your form results unchanged.

Generated Number/Order Number Add a generated number, to your forms, that increments after every form transaction. This number can be used as an order number that helps you uniquely identify your sales transactions.

Conditional Logic Create forms that can show or hide fields based on your client's responses.

Spreadsheet Style Calculations Add calculations, that compute values in real-time, to any text field on your form. These calculations are implemented in a similar fashion to any spreadsheet program currently available.

Database Field Level Encryption Encrypt selected sensitive data (like social security no & credit card no) while it resides on our databases. Not to be confused with SSL (which we also provide), database encryption protects your data after we receive it as opposed to during it's transmission.

PayPal Interface Accept payments from your customers, without any manual intervention and without additional costs, by interfacing your order forms with PayPal. Setup takes five minutes and there are no additional charges.

Authorize.net Interface Accept payments from your customers and without any manual intervention by interfacing your order forms with Authorize.net. A merchant account and an Authorize.net account are required for this option.

Stripe Interface Stripe allows the automation of payment processing in a very seemless, transparent and PCI compliant way. Your customer will be redirected to a Stripe page (to enter credit card info), without realizing they have left your site. Only Stripe will have access to this credit card info, keeping your process PCI compliant.

Form Results

Secure Login View/Download your form results securely.

Form Results Reporting(Client Data) View multiple form result records, at the same time, in a horizontal format, with form element labels for column headings. Also, view form results in a full page format one record at a time.

Global Inbox (Form Results) View your form results, across all the forms in your portfolio, in one list.

Download Results Download the collected information in an MS Excel, CSV or text format.

Customized Full Page Reports Our "Full Page Report" is a formatted report showing only one transaction per page, typically in a similar format to the form that submitted the data. In an email system, our "Full Page Report" would be equivalent to viewing the body of an email after opening it. The "Full Page Report" can be manually changed. Functions include: 1. Changing fonts, font sizes, bold, underline & italics. 2. Removing items completely from the report. 3. Moving items around on the report.

Advanced Search Feature Search for specific form results by any field on your form.

Edit Form Results By flipping a switch you can make your client's form results modifiable by you.

Delete Form Results Delete your form results one at a time.

Bulk Delete Form Results Delete your form results all at once.

Form statistic reporting keeps you informed about your client's form results utilization.

Additional User Accounts Set up subordinate (child) accounts to have form results viewing and/or delete rights to the forms of your choosing.

Mobile Friendly Format is a web form page that comfortably fits in the small screen of most mobile devices. When Auto-Detect is enabled, control is immediately passed to our Mobile Friendly Format page, when the system automatically detects the device being used is a mobile phone.

Embedded Form Plugin Allows you to embed our form into one of your existing web pages.

Download form HTML File Allows you to upload our form to your site in it's entirety, as created, by you on Elbowspace.

Data Collection Setting Choose to store your form results on our databases without any notification; or choose to store your form results on our databases along with an email notification; or simply choose to have the form results emailed to you directly.

Passing Form Values Prefill any text field on your form by passing form values in the link. No setup required and there are no additional charges. Simply add the field names, along with the values, to the URL (web address) string.

Multi-Page Form & Conditional Branch After filling out an initial form, this function will direct your client to another form, in your portfolio, based on the answer(s) to any particular question(s) on the form. The reporting will encompass all of the data entered on each form, as one transaction, on a single report. This option allows our members to create beautiful/intelligent multi-page forms. This in turn shortens the length of each individual form, helping to keep the interest of the user. The conditional branch option allows you to tailor form pages to specific answers, showing less irrelevant material to your users.

No Hidden Email AddressesHidden email fields are not required, thwarting spammers from getting your email address.

Verification Page Optional form verification page allows your clients to review their form entry before submitting. There is a choice between our standard format verification page or one you can customize/design and upload yourself.

Thank You Page A "Thank You" page is the page your clients are directed to after a successful transaction. 1) You have the option to use our default "Thank You" page which uses the images from your regular form. The text is modifiable. 2) You also have the option to have your clients directed to a "Thank You" page on your own server. 3) The third option is to have your clients directed to another form in your portfolio. 4) The final option is to upload your own custom "Thank You" page to our servers (Along with images). Our default and uploaded "Thank You" pages also include the option of displaying the detail of the current transaction (your client's entry).

Save & Retrieve Form Data This feature allows your client to save form results, to be completed at a later time. They will be prompted to enter a userid & password, which they will enter later, when they are ready to resume the form entry. It can also be used to populate a new form with your client's most recent submission.

Third Party Interface (API) This feature allows the automatic transfer of form data to applications like Paypal, Google Spreadsheets, Authorize.net, Quickbooks Online or any online software, in real-time, without having to use manual import/export functions.

Security

Standard SSL 256 Bit Security All information entered by your clients is protected by the highest level of security; SSL 256 bit encryption. Perfect for credit card & other sensitive information. This protection comes standard on all form subscription packages.

Pages Secured by SSL:

Secure Images Give your users confidence by keeping your form page 100% secure. When images resolve from an unsecured server, the browser will generate a message, to your users, similar to the following: "This page contains both secure and nonsecure items". Having us host your images and form page (on our 100% secure servers) eliminates these annoying messages. There is no extra charge to host your images.

Password protected forms allow you to pick & choose who will submit your forms.

Display IP Addresses Form results (both email & database) display the ip address of the submitter.

Block IP Addresses Block an individual ip address or a range of ip addresses, to prevent unauthorized (spam) use of your forms.

Anti-Spam Security Code Image (Captcha) Prevent unauthorized (spam) use of your forms by forcing your clients to enter the correct security code. The code is displayed on the form as an image that is not readable to automated programs.

Onsite and Offsite Backups Backups of the entire system are taken every evening.

Database Field Level Encryption The database encryption option will encrypt selected sensitive data (like social security no & credit card no) while it resides on our databases. Not to be confused with SSL (which we also provide), database encryption protects your data after we receive it as opposed to during it's transmission.

Enhanced Security Mode Enhanced Security Mode, when switched on, will send intrusion alert emails to the owner of the account, if the following conditions present themselves: a) If 5 consecutive unsuccessful login attempts cause a lockout of this account, an email indicating this possible attempted intrusion will be sent to the account owner. b) If a successful login is detected from a ip address that is not the normal location of the account owner, an email indicating this possible attempted intrusion will be sent to the account owner. Also, when switched on, will also require a password change every 90 days without repeating any one of your last 5 passwords.

Audit Trail Logs Audit trail logs provide detailed information pertaining to the access of each of
your Elbowspace.com accounts, including child (subordinate accounts).
The information includes:

Log Date: Date of login.
Log Time: Time of login.
User IP Address: An IP address is the unique identifier of a computer connected to the internet (a lot like a phone number). This information is important for recognizing a non-pattern login. This could expose a login intrusion.
User Login: Elbowspace.com userid (your email address)
System Status: Either login successful or attempted login unsuccessful.

Notifications

Auto-Confirmation Email (auto-responder) is a notification or response sent to your users, when they submit a form. It can be used as a purchase receipt, a thank you, a notification that information was received or to communicate any message. This feature also allows you to display information entered on the form, back to your users. Customize your Auto-Confirmation Email to best suit your communication needs.

Clent Email Reinder is a notification automatically sent to your clients to remind them of an appointment, event and/or reservation. The email can be sent 24 hours, 48 hours and/or 1 week prior to the event. This courtesy will help minimize missed appointments/events/reservations.

Form Results Notification Email is an email sent to you, letting you know a form was submitted. These emails can be sent per occurence or once daily, it's your choice.

Other features

Form Submission Limits A Form Submission Limit shuts down your form based on specific criteria of your choosing. ( e.g. 'class full', 'item sold out', 'this offer is limited to the 1st 100 applicants' )

Subordinate (Child) Account(s) Set up subordinate (child) account(s) to have form results viewing and/or delete rights to the forms of your choosing.

No advertisements on your form pages (with the exception of "Free Edition" forms).

No contracts or long-term commitment. However, there is a 3 month minimum on monthly subscriptions.
